Consultations:

 

Initial consultations are completely FREE at Chin Up! Aesthetics! Depending on the schedule we can do the same-day treatment as well.

 

Appointment Cancellations:

 

In order to be respectful of the needs of other patients, we ask you to be courteous and call our office if you are unable to show up for an appointment. You will be given a reminder 24 hours prior to your appointment – That would be the best time to let us know if you are unable to make the appointment.

 

Cancellations under 24 hours will be assessed a $75 cancellation fee regardless of the reason and may require a deposit on future appointments. Appointments are in high demand, and your early cancellation will allow other patients to get in for treatment.

 

How to Cancel Your Appointment:

 

To cancel your appointment, please text or call our office at 678-846-2420. If you cannot reach our Guest Services Specialist, please leave a detailed message or text us and the time will be recorded. If you would like to reschedule your appointment, please leave your name and phone number so we can reach back out to you.

 

No Show Policy:

 

A “No-Show”, is a patient who misses their scheduled appointment without any notification. A failure to be present at the time of a scheduled appointment will be recorded in the patient’s chart as a “No-Show” and a $75 charge will be implemented. We may require a deposit if you choose to reschedule the appointment. The deposit will be counted as a retainer towards future appointments. This includes arriving 15 minutes after your scheduled appointment.

Refunds Policy:

 

Select Skincare products may be returned within 10 days if unopened. A receipt of purchase will be required in order to return the merchandise. Treatments, packages, and gift cards are non-refundable.

 

If you no longer wish to complete a package series, any remaining funds may be transferred towards another service at the discretion of Chin Up! Aesthetics. No cash or charge refunds will be given. PER REGULATIONS, WE CANNOT ACCEPT ANY RETURNS ON MEDICAL ( RX) PRODUCTS). PLEASE ASK YOUR PROVIDER IF YOU HAVE ANY RETURN QUESTIONS PRIOR TO PURCHASE.

 

We do not offer refunds on ANY services rendered for any reason.

 

Once services are purchased, they will not be refunded, however, to ensure our clients always receive the greatest experience at Chin Up! Aesthetics, unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service at Chin Up! Aesthetics within 12 months of the original purchase date.

 

All injectable & service treatment sales are final; refunds or credits cannot be offered once treatment is completed.

Chin Up! Aesthetics - VIP Plan Membership Agreement

This Chin Up! Aesthetics - VIP Plan Membership Agreement (“Agreement”) is made by and between Chin Up! Aesthetics and you (the “Member”). As part of Chin Up! Aesthetics - VIP Plan, you are entitled to various benefits and service discounts, which are described below. Membership is voluntary and contingent on the payment of a monthly fee or other payment terms that may be established by Chin Up! Aesthetics, and subject to the cancellation terms included in this Agreement.

Membership Fees and Membership Levels/Benefits

Your membership level and membership fee will be charged monthly in advance.

I hereby authorize Chin Up! Aesthetics to charge my credit card provided at the time of my membership (or such valid credit card that I provide to Chin Up! Aesthetics subsequent to my enrollment as a member) in lieu of presenting it for any services, retail items, or memberships upon my request. I understand that my monthly membership fees will be charged monthly in advance, with the first monthly payment due on the date of enrollment as a member and then on that same day in each successive month.

Membership Eligibility

Membership is open to individuals aged 18 and above. Membership is non-transferable.

Transferability and Cancellation 

This membership is not negotiable, transferable, or cancelable except as otherwise provided herein. Notice of intent to cancel by the Member may be given at any time, provided that Chin Up! Aesthetics will not provide any refunds with respect to membership fees paid with respect to periods ending after notice of cancellation is given and any unpaid amounts will be charged to your credit card on file with Chin Up! Aesthetics. If a membership program has a minimum enrollment period (i.e., more than one month minimum), your membership is not cancellable until that monthly minimum period has expired. If you are enrolled in a month-to-month membership program, your cancellation requires 30 days’ notice prior to the day of the month that your membership was originally effective. If Membership is canceled prior to the completion of the 6-month period required to receive the free Botox/Dysport area and free lip filler, Members forfeit their eligibility for these complimentary treatments.

To cancel, you may (a) hand-deliver to the Chin Up! Aesthetics studio where you receive services; (b) mail written notice of your cancellation to 2914 Cherokee Street Northwest, Kennesaw GA 30144 (Suite 1A) (Attention: MEMBERSHIP CANCELLATION REQUESTS); or (c) email cancellation notice to [email protected]. If you: (a) deliver your cancellation via an email to [email protected], you must indicate “MEMBERSHIP CANCELLATION” in the subject line, be sure to get an email response from an official at the Chin Up! Aesthetics acknowledging the cancellation, or (b) hand-deliver or mail your cancellation, be sure to get a signed statement or return email confirmation from an official of Chin Up! Aesthetics, acknowledging your cancellation.

Pricing Updates 

Chin Up! Aesthetics reserves the right to change its pricing and the benefits associated with membership at any time. Chin Up! Aesthetics will give you 30 days’ advance written notice of any change in the pricing or benefits described above. Notices may be given by email, posted in the studio, or may otherwise be announced on the applicable Chin Up! Aesthetics membership program landing page on the Chin Up! Aesthetics website.

Termination by Chin Up! Aesthetics:

Chin Up! Aesthetics reserves the right to terminate or refuse to renew your agreement for any reason not prohibited by law, including, but not limited to, an unsatisfactory payment history. Chin Up! Aesthetics reserves the right to collect at any time any delinquent or outstanding balance(s) that has not been paid for any services provided or monthly payments owed.

Limitation of Liability

Chin Up! Aesthetics shall not be liable for any damages, losses, or injuries arising out of or related to Membership, except to the extent caused by Chin Up! Aesthetics' gross negligence or willful misconduct.

**NOTICE:** We are hereby informing you that should you (the Member) choose to pay for any part of this Agreement in advance, be aware that you are paying for future services and may be risking loss of your money in the event Chin Up! Aesthetics ceases to conduct business in the location where you primarily receive services, or at all. Chin Up! Aesthetics does not post a bond, and no other protections may be provided to you should you choose to pay in advance.

Governing Law

These Terms shall be governed by and construed in accordance with the laws of Atlanta.
